THIS IS A DISCOUNT PLAN, NOT A DENTAL INSURANCE PLAN.
Exclusions Apply:
- It cannot be used in conjunction with another dental plan, dental insurance, or financing program such as CareCredit.
- This plan is only honored at Sharp Family Dentistry. It cannot be used at any other dental or specialist office.
- Patient’s portion of bill is due at time of service.
- No refunds of premiums will be issued at any time if participant decides not to utilize the dental plan.
- All fees are non-refundable.
